Discharge to access carer survey
If someone you care for has recently been discharged from either South Tyneside District Hospital or Sunderland Royal Hospital they would like to hear from you.
South Tyneside and Sunderland NHS Foundation Trust (STSFT) are looking to gather the views of carers who have experienced their discharge process. The findings will help them understand what is good about patient care and whether any improvements are needed. They would be very grateful if you could take the time to complete their survey.
