Your views are sort on palliative and end of life services in South Tyneside and Sunderland

The North East & North Cumbria Integrated Care Board (ICB) would like your views on palliative and end of life services in South Tyneside and Sunderland. These are a broad range of services supporting quality of life for people with illness that is not curable. For example:

  • Hospices
  • Hospitals
  • Counselling services
  • Nursing and care homes
  • Voluntary, Community and Social Enterprise services e.g. Macmillan or Age UK
  • Community services e.g. a nurse that visits the home
  • GP practices

 

They  know that this will be a difficult period for you. They are grateful if you're willing to take part. However, involvement is entirely up to you, and you can stop at any point.   If you would like to look at different ways of getting some extra support, there are some links to information at the end of the survey.

They want to hear from

  • people who have received palliative or end of life services, and 
  • family members, friends and carers. 

Your views will be part of an NHS review which will help show what works well and what could be better. You can pass this survey to someone else if you think they are more able to answer the questions. 

You can get involved by filling in a survey. The questions are about your general experience, not one specific service. Please fill out the survey online if you can.

The survey will take around 15 minutes to finish. The closing date is 18th February 2024.

How they will use the answers you give them

The NHS North East North Cumbria Integrated Care Board will gather the feedback and write a report. They don't need you to tell them anything that will identify you. The report will be anonymous and will feed into the review of services. Your service provider will also get a report so they can share results. They will tell your service provider what actions will be taken as a result of the review and ask them to share, but this may take a little longer.
